Repair Request

Use the form below to pre-register your repair in our system. Once submitted, you will be given further information and instruction for your repair. If you have any questions regarding the process, please contact us.

  • We're pleased to announce that you can pre-pay for your repair with a Credit Card or PayPal. Please note that in most cases, you are paying for the minimum bench rate, and additional charges may apply after equipment evaluation.

  • Please be aware that warranty claims only cover manufacturer defects. For details, consult the warranty documentation for your product.

    If the problem is found to not be covered under the manufacturer's warranty, our standard rates will apply. If the repair is not covered under warranty and you choose to not have the unit repaired, a $55 diagnosis fee will apply.

    A dated receipt showing the unit's serial number is required for all warranty repairs. If you have an electronic copy, it may be attached and uploaded at the bottom of this form.

  • Please check your Stripe API Settings. Click the "Connect with Stripe" button to use Stripe.
  • If you can pay now, please select one of the Pay Now options. Use this Pay Later option only if one of the listed method applies. Note that we may not begin your repair until payment is received.

  • If you have a PO or Reference Number you would like to associate with this Repair Request, please enter it here.
  • We will bill a pickup fee based on your location.

  • Since we don't know the size and weight of your item, we will bill you return shipping after completion. Alternatively, you may include a return shipping label and no additional handling or shipping fees will be charged.

  • We will bill a delivery fee based on your location.
